Travel Immunisation Clinic – offering free travel advice

Travel Clinic – This free clinic is run by our practice nurses. Prior to attending the clinic you will be asked to complete our Travel Questionnaire form here. The form can be downloaded from our website or collected from reception.

When you have completed the form please scan and email it back to us at carepoint@gp-E86618.nhs.uk or leave it at reception.  The nurse will look at your form and telephone you if you need to have any immunisations or travel advice.  You will only require an appointment if the nurse recommends it.

The Carepoint Practice is a designated Department of Health Yellow Fever Vaccination Centre. We provide a travel vaccination service, offering protection and cover against yellow fever, meningitis, rabies, hepatitis A + B, typhoid and tetanus. Up-to-date advice is given for appropriate anti-malarial protection. Most of these services are free to our patients, however, there are a few exceptions to this not covered by the NHS.

If you are visiting an EEC country you will need to obtain a European Health Insurance Card (EHIC) which replaces the form E111. EHIC can be obtained by logging on to www.dh.gov.uk/travellers, or by telephoning 0845 606 2030.
