"Lead by Example"
Appointments
Requests for help and appointments are triaged via our online health questionnaire tool called Patchs. You are able to submit an Patchs request from 08:00 Monday - Friday (excluding bank holidays) whilst there is capacity for regualar appointments. You will then be contacted regarding your query within 24 hours (one working day) of receipt of your Patchs request. We have introduced Patchs so we are able to efficiently manage patients without the need for the patient to come in.
If you don't have access to Patchs via any means, then our receptionist will use a similar Patchs questionnaire to gain the clinical information required for our Clinicians to triage your request. This will involve you giving details of the problem to help identify the most appropriate pathway. All telephone calls are recorded for monitoring and training purposes
Extended Hours Hubs
There are 3 hubs spread across the borough - Eastcote Health Centre (North); Uxbridge Health Centre (Central) and HESA centre (South) whereby practices can book their patients but there is limited availability. The service is open Monday to Friday from 6:30pm-8pm and 8am-8pm on weekends (between 3 hub sites). Click here for further Information
Missed Appointments: Every appointment in the NHS is valuable. If you cannot make an appointment it is essential that you contact the Practice or the Hospital as soon as possible so that your appointment is not wasted and is given to someone who really needs it.
