Patient Access

At Carepoint Practice, we aim to offer everyone in our care a high quality service and therefore we have made patient access available to all our patients.


With Patient Access, you can now access our services (mentioned below) at home, work or on the move — wherever you can connect to the internet. What's more, because Patient Access is a 24 hour online service you can do this in your own time, day or night.

  • Book an appointment.

  • Order repeat prescriptions.

  • Change your address details.

Available through the Patient.info website and as a mobile app for both Android and iOS devices, patients can easily access our services and use it from virtually anywhere.

About Patient Access

GP online services gives patients the option to book appointments, request repeat prescriptions and view their GP records, using their computer, tablet or smartphone rather than having to phone or visit their practice.

 

Online services complement the existing ways patients can access appointments, prescriptions and their records. Our practices already have the facility to offer online access to records and transnational services for our patients.

If you are not already registered for GP online services please let a member of the practice team know you would like to use this service.

Using online services makes it more convenient for people at our practice, as there’s no need to make a phone call or visit us. Computer, smartphone or tablet users can make or cancel an appointment at a time that suits them, even when the practice is closed.

Please Note: If you already have 2 appointments and cannot book more, this is because you are able to book a maximum of 2 appointments on-line at any time. This is to avoid abuse of the online booking system. If you need to book more than 2 appointments, you will need to either call the practice (see ‘Contact Us’ section of our website), or cancel one of your existing 2 appointments and rebook your desired appointment slot.

Prescriptions page

The easiest, safest and quickest way to order your repeat prescription is online. There is no need to make a phone call or visit us; instead computer, smartphone or tablet users can request repeat prescriptions anywhere – 24 hours a day, seven days a week. You can also check what medication you should be taking and when.

How Do I Register? You can register in three ways: 


1. Click Here to download our registration form and find out more information about how to register & email it to

    hillccg.carepoint@nhs.net with a valid ID attached.


2. Email hillccg.carepoint@nhs.net with your name, date of birth, post code & proof of identity (drivers license or passport)


3. Attend Carepoint Practice with valid ID and a receptionist will be able to provide you with your login details if all criteria outlined is met.

Important Information – Please read before signing up

If you wish to, you can now use the internet to book appointments with a GP, request repeat prescriptions for any medications you take regularly and look at your medical record online. You can also still use the telephone or call in to the surgery for any of these services as well. It’s your choice.  It will be your responsibility to keep your login details and password safe and secure.  If you know or suspect that your record has been accessed by someone that you have not agreed should see it, then you should change your password immediately.

 

If you can’t do this for some reason, we recommend that you contact the practice so that they can remove online access until you are able to reset your password.  If you print out any information from your record, it is also your responsibility to keep this secure.  If you are at all worried about keeping printed copies safe, we recommend that you do not make copies at all.

 

Before you apply for online access to your record, there are some other things to consider.

Although the chances of any of these things happening are very small, you will be asked that you have read and understood the following before you are given login details.

 

Choosing to share your information with someone

It’s up to you whether or not you share your information with others – perhaps family members or carers. It’s your choice, but also your responsibility to keep the information safe and secure. Proxy access refers to access to online services by somebody action on behalf of the patient and usually with the patient's consent.

Coercion

If you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time.

Misunderstood information

Your medical record is designed to be used by clinical professionals to ensure that you receive the best possible care.  Some of the information within your medical record may be highly technical, written by specialists and not easily understood. If you require further clarification, please contact the surgery for a clearer explanation.

Information about someone else

If you spot something in the record that is not about you or notice any other errors, please log out of the system immediately and contact the practice as soon as possible.
