As part of Carepoints Commitment, in line with NHS England & the CCG to improve/upgrade our IT infrastructure, we have removed Magic Form Builder submission from our website.
We advise all patients to sign up to Patient Online Access to request medications (please call reception on 01923820866 after 10:00 for more information). You are able to request medications 24 hours a day & track your request in real time with your GP surgery.
Alternatively, please email in your request to carepoint.prescriptions@nhs.net - However this method may be altered in the future.
If you have any queries regarding your medications or have any issues you would like to raise with your GP (clinical or administrative), please use our new eConsult triage system to contact us. You are able to use this system 24 hours a day & we will contact you with a response within 1 working day of us receiving the request.
